The completed Transcript Request Form (link is below) can be dropped off at Eisenhower, mailed via the USPS or emailed. Please include your check or money order, payable to Eisenhower MS/HS. You may also make a credit card payment through our webstore.  The completed Transcript Request Form and fee must be received before the request is processed. Current students:  First ten requests are sent free of charge.
